Meet Anna
In over 20 years as a registered nurse, Anna Petrey has established a reputation for high-quality, personalized care. She first began her career as an EMT, later becoming an RN and then a Family Nurse Practitioner (FNP-C.) in Emergency Medicine.
She started Streamline Health & Wellness in 2019 as a way to serve patients in the comfort and privacy of their homes. All of her services are customized, unique and specific to the patient.

What insurance do you accept?At this time, I do not accept any type of insurance. I do not bill insurance. At your request I will provide a copy of your bill (super bill) which you may submit to your insurance company for reimbursement. I cannot provide you with any specific information about the amount of reimbursement your insurance may provide.

Do you provide Emergency and After Hours service?I do not provide emergency care. This means I am not always available during evening and weekend hours, but I will try to accommodate you if I can. I do not provide in-patient care during hospitalization. (However, you are free to call me and inform me of what is going on and I would love to be your advocate in any way I can). I request that you maintain a relationship with a primary care provider or institution which provides emergency medical services.

Do you provide Primary Care services?Although I do provide annual wellness exams including basic lab work, I do not provide the standard required preventative measures like mammograms, colonoscopies, vaccination, etc. Please maintain a relationship with a primary care provider or institution which provides these services.

What are your Services & Prices?All services are priced individually with a moderately priced house call fee. Additional services such as IV fluids, medication administration, testing, aesthetics, testosterone, and medical weight loss are at an additional cost. Membership plans are offered as well for your convenience.
